MODS CLINIC · Anti-Aging Self-Assessment Service

MODS CLINIC ("the Clinic") processes users' personal information as described below in connection with the anti-aging self-assessment service ("the Service").

1. Personal information collected

The Service collects personal information differently depending on the stage of use, as set out below.

(1) When using the self-assessment

We collect age range, gender, country of residence, and responses to the 16 self-assessment questions.

(2) When requesting a consultation

We collect your name, contact number, and email address (optional).

(3) When using the in-clinic pre-visit questionnaire (for patients with a booking or an active consultation)

For patients whose visit has been booked or who are in an active consultation, we send a personalized link to a pre-visit questionnaire. Through this questionnaire we collect the patient's name (or another identifier such as a messaging-app ID), contact details, internal reference number (MCMS lead/patient ID), responses to the 16 questionnaire items, and the AI-generated analysis result. Unlike the self-assessment in (1), this information is collected and stored in a form that identifies a specific individual from the outset.

2. Outsourcing of personal information processing

To operate the Service, the Clinic outsources certain personal information processing tasks to the following infrastructure providers.
